Dropsy Treatment — Step-by-Step Protocol
Act fast. Every day of delay reduces the chance of recovery. Follow this protocol in order.
Step 1 — Set Up a Quarantine Tank
Use a 5-10 gallon hospital tank with a bare bottom (no gravel). Set up:
- Sponge filter or gentle air stone — no strong current
- Heater set to 78-80°F (same temperature as main tank)
- No plants or decorations — reduce hiding spots so you can monitor the fish

Alternative: API Fin & Body Cure (minocycline). Use 1 packet per 10 gallons, repeat after 24 hours and again after 48 hours. This is a good option if Kanaplex is unavailable or if the fish hasn’t responded to kanamycin after one treatment cycle.
Treatment course: 7-10 days minimum. Do not stop treatment early even if the fish appears better — stopping early can cause antibiotic resistance.
Critical: Remove activated carbon from the filter before medicating — carbon absorbs medications and renders them ineffective.
Step 4 — Supportive Care
Supportive treatments improve the fish’s chances of recovery:
- API Melafix — 5 mL per 10 gallons daily. This natural tea tree oil remedy provides antibacterial support for the quarantine tank. Use it as a supportive treatment alongside stronger antibiotics, not as a replacement.
- API Pimafix — 5 mL per 10 gallons daily. Prevents secondary fungal infections that often develop on weakened fish during recovery.
- Raise water temperature to 78-80°F to boost metabolism (same as main tank temp)
- Offer small amounts of high-quality food — garlic-infused food can stimulate appetite
- Perform 25% water changes every 48 hours (re-dose medication after each change)
- Keep the quarantine tank dark to reduce stress
Step 5 — Monitor and Adjust
Check for improvement every 24 hours. Signs of recovery include reduced bloating, scales flattening, and the fish becoming more active.
If you see no improvement after 5 days of treatment, consider switching antibiotics. For example, switch from Kanaplex to API Fin & Body Cure or vice versa.
When to consider humane euthanasia: The fish has stopped eating for 7+ days, lost the ability to swim normally, shows full body pineconing with bulging eyes, or shows no improvement after two complete treatment cycles. Consult a fish veterinarian for guidance on humane methods.
Dropsy Prognosis
